ICD-10-PCS Codes and Guidelines

The latest ICD-10-PCS codes and guidelines for the fiscal year 2024 have been unveiled by the Centers for Medicare & Medicaid Services (CMS). With a net growth of 73 codes, the total count of inpatient procedure codes has reached 78,603. Notably, the New Technology Section has experienced the largest surge, introducing 40 new codes. In addition, the Body Part Key has undergone expansion to encompass the appendiceal orifice for the appendix, the intracranial portion of the middle meningeal artery for the intracranial artery, and the chin for subcutaneous tissue and fascia of the face. It is worth noting that the Body Part Key serves as an appendix for ICD-10-PCS.

New additions have been made to the Device Key, incorporating the following devices:

  • EV ICD System (extravascular implantable defibrillator lead)
  • Aveir™ VR, available as a single chamber
  • Flourish® Pediatric Esophageal Atresia Device
  • Aortix™ System
  • Vanta™ PC Neurostimulator
  • Intellis™ Neurostimulator

These devices are now included within the updated Device Key.

Under the Substance Key, a noteworthy addition is the inclusion of Plazomicin in the category of Other Anti-infective. Furthermore, Angiotensin II, GIAPREZA™, and Human Angiotensin II (synthetic) have been incorporated into the Vasopressor category.

As for the New Technology Section, there are numerous changes, too numerous to list individually. These changes strongly imply that there will be a significant number of modifications in the New Technology Add-On Payment (NTAP) for the year 2024.

Additionally, there are six new tables introduced in the New Technology Section of ICD-10-PCS, namely:

  • X05 (New Technology Section, Nervous System, Destruction)
  • X2H (New Technology Section, Cardiovascular System, Insertion)
  • X2U (New Technology Section, Cardiovascular System, Supplement)
  • XNR (New Technology Section, Bones, Replacement)
  • XV5 (New Technology Section, Male Reproductive System, Destruction)
  • XX2 (New Technology Section, Physiological Systems, Monitoring)

In response to public input and internal evaluation, revisions have been made to the ICD-10-PCS guidelines. Guideline B5.2b, which covers the percutaneous endoscopic approach with hand assistance or extension of incision, has been updated to explicitly include hand assistance and the temporary exteriorization of a body structure.

Similarly, another guideline, B6.1a, the General guideline, has been modified by removing the example involving inadequate device size or documented complications.

A comprehensive review of the ICD-10-PCS codes, guidelines, and New Technology Add-On Payments (NTAPs) will take place on Talk Ten Tuesdays on August 16, 2023, offering more detailed insights into these updates.
